An incident that results in (or could have resulted in, in the event of a “Near Miss”) property damage, injury/illness, or death.
A reportable incident covers:
The death of a participant
Serious injury of a participant
Abuse or neglect of a participant
Unlawful sexual or physical contact with, or assault of, a participant
Sexual misconduct committed against, or in the presence of, a participant, including grooming of the participant for sexual activity
The unauthorized use of a restrictive practice concerning a participant.
Concerning Incidents
We consider your safety essential and work very hard to deliver safe services. But mishaps do occur. Sometimes, people behave unfairly and make mistakes. These things happen and are called “incidents.” This form describes our efforts to prevent incidents and what we do if one does occur. Additionally, you can help to avoid incidents. Be cautious with your actions. Show consideration for those in your vicinity. Additionally, let someone know if you don’t feel comfortable.
Being safe and feeling safe
To keep people safe, NDIS providers are required to follow specific rules. We try our hardest to ensure everyone’s safety. This applies to you, our staff, and other community members. We talk about what leads to mishaps and how to prevent them. It is known as “risk management” in this context. We inquire about you and consider the services we offer, the settings in which they are presented, and the people who collaborate with you and others in your surroundings. Everyone should feel safe and comfortable. You can let us know if something makes you uncomfortable.

The death of a participant
If there is an issue or an accident, our employees know what to do.
If an incident occurs, we follow the NDIS guidelines.
We need to inform the NDIS Commission.
We need to look into the situation.
We must act to ensure this tragedy does not occur again.
All of this must be discussed with the injured individual.
We must follow these regulations if someone is harmed, claims to have been hurt, or employees maltreat NDIS participants.
Tell someone else if you feel uncomfortable talking to our staff. You can discuss it with your loved ones, friends, and an advocate. The NDIS Commission establishes the guidelines and supports participants who violate them and is the appropriate party to submit a complaint with.
What is an Incident?
Everyone is entitled to be treated equally. You have a right to know what went wrong and what steps are being taken to make things right if you are a victim of a crime. You are entitled to file a complaint if you do not receive these responses. We will always keep you informed. We wish to treat you with dignity. You can ask for help if you’re unhappy with our complaints process. You might get assistance from the NDIS Commission. Additionally, an advocate can assist you by advocating on your behalf.
